The NHS Long Term Plan laid out the vision for how integration will support a more proactive approach to care delivery. And COVID-19 has made the need for more coordinated care based on data even more urgent. It’s about population health: using the collective resources of the local system, NHS, local authorities, the voluntary sector and others to improve the health of local areas.

PHM is the key that will allow integrated care systems (ICSs) to transform into ‘learning health systems’ by building their capabilities to track and improve outcomes for key population segments across time. Optum has the firsthand knowledge and experience to support systems on this journey.

Optum was founded on a single principle: data and analytics can make health systems work better for everyone. Our approach to PHM is built on our experience using the world’s largest health care data set to drive care transformation.

With real-world experience managing risk-based health systems, our ambition is to bring those PHM capabilities to local systems to integrate care at every level and improve care delivery. 

As NHS England and NHS Improvement's National Development Partner for PHM, we have worked with systems across the UK to develop the necessary infrastructure, intelligence and interventions that have delivered real impact.

We leverage our expertise running data-driven, risk-bearing primary care networks in the US to support ICSs with the tools and transformation support that can ultimately improve quality and outcomes for the patient and value and experience for the provider.
